An incisional hernia is a bulge in the abdomen that forms when tissues or organs protrude at the site of a weak surgical scar. It can occur when the surgical incision weakens the abdominal muscles which normally support the abdominal organs or the muscles give way due to increased intra-abdominal pressure.  It occurs after complications of abdominal surgery.  The bulge increases with the passage of time. The hernia may become irreducible or the internal organ like the intestine may get trapped and obstructed.
